Who Is Eligible For Hospice Care

Potential hospice patients may be identified by a diagnosis, symptoms, or a combination or both.  The following sections are meant as a guide to help determine if a patient is hospice appropriate.  Please call Admissions at (936)632-1514 to discuss eligibility.  If your patient does not meet Hospice Medicare Criteria, they may be eligible for our Transitions Program.

Signs and Symptoms to Look for

May include one, or a combination of the Following: 

  • Diminished Functional Status
  • Increased weakness, fatigue, drowsiness
  • Decreased appetite
  • Progressive weight loss
  • Progressive decline despite medical therapies
  • Multiple Hospitalizations/frequent ER visits
  • Pain that is more difficult to control
  • Uncontrolled nausea/vomiting
  • Increasing dyspnea
  • Dysphagia
  • Oxygen dependency
  • Ascites
  • Recurrent infections
  • Decline in mental status

Diagnoses That May Qualify

  • Malignancies (all forms of cancer, leukemia)
  • End Stage Pulmonary
  • End Stage Heart Disease
  • End Stage Alzheimer’s Disease/ Dementia
  • CVA (Cerebrovascular Disease)/ Stroke
  • End Stage Acute Renal Disease
  • End Stage Chronic Renal Disease
  • End Stage Liver Disease
  • ALS (Lou Gehrig’s Disease)
  • Parkinson’s Disease
  • End Stage HIV Infection/ AIDS
  • Multisystem Failure/Debilitating Chronic Conditions
